If you have an insurance company listed above:
We will collect any co-payments and charges from you at the time of service. Our office staff will file a claim for services to your insurance company and expect to be reimbursed by your insurance company. Payment from your insurance company can’t be guaranteed, and benefits are not determined until your claim is processed. You will be financially responsible for any claims your insurance company denies.
Payment Information
We accept Visa MasterCard, Discover, cash and personal checks. For our patient’s convenience we have arranged a payment plan through CareCredit that helps our patients receive their treatment in a timely manner.
